Our Story & Motto

Welcome to Wakefield Health Care Center, where community, care, and comfort come together. Located in Wakefield, Nebraska, we are a community-owned assisted living and long-term care center, proudly certified for Medicare, Medicaid, and VA benefits.

Our community is home to a variety of services, including PMC Outpatient Therapy, Independent Ash Street Apartments, Assisted Living, and our Nursing Home. At the heart of it all is our beautiful courtyard—a peaceful space that connects our Assisted Living and Nursing Home, offering a place to relax and enjoy the outdoors.
